I have a 1996 Ford F-350 dual rear wheel 2-wheel drive 4-door diesel 5-speed that pulls to the right. I've had the toe checked twice and no problem there; brakes and bearings aren't dragging, tires are new, steering gear mesh load has been tightened (which helped a bit but didn't fix all of it). I'm out of ideas, what else could it be?

First, try swapping the tires from side to side to see if that helps. Check the caster from side to side. The caster may be within spec on each side, but the total caster or "cross caster" can cause the vehicle to pull. Also, check the condition to the pitman and idler arm bushings on this model. Deflection can cause changes in toe while the vehicle is moving.
